How to know what exactly was changed in LMDDocument?

I need to know what the line numbers have been changed in the event OnDocumentModified.

Generally, there no way. If you absolutely need it, you have to store a "previous" version of the text and compare it with the new one manually.

I've added OnLinesChanged(AFistLine, ACount: Integer) event to the edito document. Edit's internal parser use the same way to invalidate its own internal state.
